Safety waiver
Every driver and tactical participant must complete a waiver before entering the water. A parent or legal guardian must sign for a minor.
This form records acceptance for the proposed facility workflow. Final legal language must be reviewed by Nevada counsel, the facility insurer and applicable marine-safety authorities before launch.
